Keith Daly – Senior Consultant / Workplace Accident Investigator

Keith Daly brings over 20 years of experience in Occupational Safety and Health, including more than 15 years with the U.S. Department of Labor’s Occupational Safety and Health Administration (OSHA). During his tenure with OSHA, he conducted inspections across construction, manufacturing, agriculture, and general industry, gaining extensive experience in enforcement procedures, accident investigations, and regulatory compliance. He has specialized expertise in machine guarding, lockout/tagout (LOTO), control reliability standards, and workplace hazard prevention.

Keith has led numerous high-profile investigations, including fatality cases and incidents involving railcar operations, cargo safety at airports, trenching and excavation hazards, grain elevators, machine guarding, and structural collapses. His work has driven measurable improvements in workplace safety practices and helped strengthen compliance standards across multiple industries. He also provided support for complex enforcement cases. 

Before joining OSHA, Keith held safety leadership positions in both construction and general industry, where he developed and implemented comprehensive safety and health programs from the ground up. This hands-on experience gives him a deep understanding of how to align operational goals, insurance requirements, and employee safety.

Keith holds a Bachelor of Science in Occupational Safety and Health from Illinois State University and continues to expand his expertise through advanced training and ongoing professional development in emerging safety and regulatory practices.
