Report a Fatality or Severe Injury
Employers must notify OSHA within 8 hours for fatalities and within 24 hours for inpatient hospitalizations, amputations, or eye losses. If one of these serious incidents happens at your facility, time is critical. This is the official portal to get it done.
Severe Injury Reports: You can also access data on reported severe injuries, including amputation, hospitalization, or eye loss incidents.
Electronic Recordkeeping:
Injury Tracking Application (ITA): Gone are the days of filing cabinets and paperwork chaos. Use OSHA’s Injury Tracking Application(ITA) to Submit and manage your electronic injury and illness records.
Remember, the records must be maintained at the worksite for at least five years. Each February through April, employers must post a summary of the injuries and illnesses recorded the previous year. Also, if requested, copies of the records must be provided to current and former employees, or their representatives.
